Great News! Members will receive their second rebate check of 2023 in October which reflects vaccine purchases between January 1, 2023 and June 30, 2023. CPP members have been taking advantage of receiving vaccine rebate dollars via automatic bank transfer.

If you would prefer to receive your rebate via electronic deposit, please submit this ACH authorization form to CPP at cpp@nationwidechildrens.org or fax it to 614-722-2149. If you already submitted this form to CPP, you do not need to complete it again.
